Healthcare and Medical
Savings Programs: Some of UCA's benefits packages include health and medical savings programs. Several states have enacted legislation relating to being a “Discount Medical Plan Organization” (DMPO). These states require certain disclosures to be made regarding your rights and the programs to be registered. It is important to note the following terms and conditions of participation in the DMPO: These programs are not insurance nor a health insurance policy. These programs provide discounts at certain health care providers for medical services. These programs do not make payments directly to the providers of medical services. The member is obligated to pay for all health care services, but will receive a discount from those healthcare providers who have contracted with the DMPO. The corporate name and location of the DMPO is: AccessOne Consumer Health, Inc., 84 Villa Road, Greenville, SC 29615 In addition to the lists of participating providers to be found at WWW.UCACARD.COM, the DMPO will also provide the member with a list of providers at its website, www.accessonedmpo.com. The member will be billed by the provider at the discounted rate, at the time service is rendered. The member is obligated to pay the provider for services rendered. In no instance, will the DMPO make payments directly to healthcare providers on behalf of the member. These programs and the DMPO or UCA have no liability for providing or guaranteeing service or any liability for the quality of service rendered. The Hospital Negotiation Service is offered as a convenience to DMPO participants. The DMPO is not associated with, nor does the DMPO have a provider agreement in place with the Hospital Negotiation Service. Members use the service if and when they choose to and no discounts or savings are warranted by the DMPO for services rendered by hospitals or offered to members. |
