you and your family have access to a national network of over 65,000*
available dental practice locations to choose from nationwide.
Participating dental locations provide savings on average of 30%** and may
range up to fifty percent (50%) *** on some dental services, including
cleaning, x-rays, fillings, root canals, crowns, bridges and orthodontia.

The program is NOT INSURANCE. It is a member reduced-fee-for-service program
that is designed to save you money on most of your family's dental needs.
Your final cost will depend upon the type of dental work performed, as well
as your geographic location.
Important: You must pay the reduced cost for the dental work performed
at the time service is rendered unless other arrangements are made with
the dentist’s office. There are no out of network benefits or benefits
payable in this program. The Aetna Dental AccessTM discounts
cannot be combined with any other discount dental program.
How to Use this Service: To access the dental network: call the toll-free number indicated on
your membership card. For your convenience, member services is available 24 hours a day, 7 days a week,
excluding holidays. The service representative will request
your name, group number, membership number and zip code. Once your
information is confirmed, you will be provided with the names and
telephone numbers of participating dentists in your community. Please make
sure to identify yourself as an Aetna Dental AccessTM member
when making an appointment with a participating dentist.
Remember: UCA is the name on your card, but the dentist will
recognize Aetna Dental Access TM network not UCA.
If you are currently working with a dentist who does not participate
in this program, you may ask the dentist to contact us at the toll-free number
listed on your membership ID card. A provider application will be sent to
the dentist promptly. Unfortunately, we cannot guarantee acceptance by either the
Aetna Dental AccessTM network or the dentist.
Remember, since you and your family members are eligible to receive
these discounts throughout your membership, the more you use the dental
services, the more you save.
The Aetna Dental AccessTM network provides enrollees with access
to a network of independent practicing providers. All network providers
are independent contractors in private practice and are neither employees
nor agents of Aetna Life Insurance Company and/or its parents,
subsidiaries or affiliates (“Aetna”). The availability of any particular
provider cannot be guaranteed, and provider network composition is subject
to change. Aetna does not provide medical or dental care/treatment and is
not responsible for outcomes. All dental care is the responsibility of the
treating provider, in consultation with the enrollee.
Selection of a dentist is also the responsibility of the enrollee and
is not based on any representations by Aetna.
There are no benefits payable to enrollees, nor does Aetna
compensate dentists for services they may render to enrollees. Aetna is
not an insurer, guarantor or underwriter of any services provided under
the program or of any payments to providers. Enrollees arrange for needed
dental care, including payment, directly with the network provider.
Enrollees are responsible for the entire cost of the dental care and Aetna
shall in no event be liable for any payment to a provider accessed under
the program or for the refusal of a provider to accept the discounted
rates. Aetna Dental AccessTM is a service mark of Aetna Inc.
The Aetna Dental AccessTM program network is provided by Aetna
Life Insurance Company.
Note: The Dental Program is NOT insurance. It
is a member reduced-fee-for-service program that provides discounts at certain
providers. In order to receive the special pricing, you must pay in full
at the time of service and verify that the provider is an active
participant of The Aetna Dental AccessTM network.
